ABSTRACT NURSING CARE FOR MR. Y WITH CARDIOVASCULAR SYSTEM DISORDER: CONGESTIVE HEART FAILURE AT 5th FLOOR WARD IN RUMAH SAKIT ADVENT BANDUNG Arranged By: FREDERICK MAKER ANDRE NIM: 1552051 This scientific paper in the background by the prevalence of Congestive Heart Failure in Indonesia on 2016, which Indonesia was ranked second in Southeast Asia with the number 371,0 thousand people, after the Philippines that was ranked first with the number 376,9 thousand people. Based on diagnosis/symptom, the most sufferers of heart failure is in West Java Province with 96.487 people (0,3%). The purpose of this scientific paper is to apply the nursing care on Mr. Y with CHF, using a holistic care approach. The method used is descriptive method by collecting data through interview, observation, documentation, literature study, and internet study. At the assessment stage, found: the patient shortness breath, cough, edema, the skin of extremities looked pale and checked Hematology Examination, Blood Gas Analysis, and Thorax Radiology. Based on the data analysis, there appeared four nursing diagnoses: decreased cardiac output, excess fluid volume, activity intolerance, and ineffectiveness of tissue perfusion. After the author applying three days of nursing care from four diagnoses, there were two diagnoses resolved and two diagnoses unresolved. The resolved diagnoses are: gas exchange disorder and activity intolerance, and the unresolved diagnoses are: excess fluid volume and ineffectiveness of tissue perfusion. The author hope this scientific paper can be an input in providing nursing care to patients with cardiovascular disorder: CHF, and to improve knowledge to prevent unwanted things and death risk.
