ABSTRACT Along with the rapid developrnent of information technology makes the information technology has an important role in human life . In the field of employment , for example , information technology helps enable people to do the job . One example is a database application . By using a database application , all processes are complex and large scale can be simplified and speeded up . In the faculty of information technology lndonesian Adventist University 's own faculty evaluation to determine the performance of each of his teachers. But the ongoing evaluation is still done manually using a questionnaire that has many risks Yag can occur as the data is lost, corrupted . and so on . By using a web -based database applications of the risk can be eliminated because each assessment data will be stored in the database system Additionally this application can facilitate faculty in conducting evaluations Expected later this designed system can input and display the results ofthe evaluation offaculty each semester well , can enter the data query , data is faculty, student data , and the data subject well , and hclp facilitate the faculty evaluation process and get rrsults more effective assessment.
