ABSTRACT DESCRIPTION OF SELF-HEALTH MENAGEMENT OF THE CHRONIC DISEASE MENAGEMENT PROGRAM PARTICIPANTS AT PUSKESMAS PARONGPONG, BANDUNG BARAT DISTRICT WORKING AREA JEFRY NIM: 1551062 Based on the investigator’s observations when visited PUSKESMAS Parongpong, there was still lack of people’s involvement to the hypertension and diabetes mellitus patients toward the Prolanis activities. The purpose of this study was to find out the description of Prolanis participants’ self-health related to family and health workers support at PUSKESMAS Parongpong in Bandung Barat. This study used descriptive correlational method with a nonprobability sampling method. Data were obtained from filling out of questionnaire and was analyzed by using SPSS. The results of data analysis obtained p-value <0.05 which showed a weak positive relationship between family support and blood pressure (diastolic), a weak negative relationship between family support and exercise patterns, a weak negative relationship family support and consumption of salt, a negative relationship moderate between family support and means of transportation to health services, a weak positive relationship between health workers and sports patterns. The conclusion of this study were the hypertension and diabetes mellitus patients where unable to control the blood pressure and blood sugar levels, the family support was given well with percentage was more than half. Most of the health workers have provided good support to the respondents. There was a relationship between self-health with the family and health workers support at PUSKESMAS Parongpong in Bandung Barat. From this results, the investigator suggested the concern health workers to give health promotion to the respondents or the community by emphasizing that someone’s self-health is depending on he family and health workers support.
